Impact Plus Services

Intrust Healthcare has been an approved provider of Impact Plus Services since the inception of Impact Plus in 1999. Intrust has earned substantial trust and confidence by virtue of providing treatment and intervention for hundreds of families in the South Central and Eastern Kentucky regions.

  • Targeted Case Management
    An Intrust Case Manager will be the first person the client's family will have contact with in order to begin the process for eligibility into the Impact Plus program. The Case Manager will meet with the child's family and obtain all necessary information to complete the eligibility packet.
    Once the child is approved the role of the Case Manager is to:
    • Assist an eligible recipient in accessing needed medical, social, educational and other support services.
    • Maintain the child in the least restrictive environment for their needs by utilizing community resources and wrap-around services including: counseling services, housing needs, educational support and crisis planning.
    • Facilitate Service Team meetings every 60 days with that child's service professionals and family members in order to create a comprehensive service plan.
    • Provide the client's family with support and advocacy in various areas including, but not limited to:
    • Community mental health services school system and other educational services legal services.

  • Family/Collateral Therapy
    A behavioral health consultation or service planning meeting with a parent, legal representative, school personnel, or other person with custodial control or supervision of the eligible recipient. The client may be present during this service. This service will focus on the goals and objectives found in the recipient's collaborative service plan or as part of a service planning process. This service can be provided by a behavioral health professional, either individually or under clinical supervision.

  • Group Therapy
    A therapy service provided face-to-face with a client in a group setting focusing on a specific objective (eg. social skills, anger management). The treatment and interventions will focus on the goals and objectives found in the client's collaborative service plan. This service can be provided by a behavioral health professional, either individually or under clinical supervision.

  • After School and Group Summer Programs
    Intrust Healthcare offers an after-school group service, as well as a group summer program. Our group summer program runs for 3 hours, 2 days per week. Groups are tiered and broken into 3 age categories. Group focuses include: social skills, anger management, life skills, self-esteem, etc. Lunch will be provided and our staff will be happy to assist with transportation needs if necessary.

  • Individual Therapy
    One-on-one counseling service between the therapist and the child. This service can be provided in our office, at the client's home, in a school setting, or in the community. The treatment and interventions used during the session will be focused on the goals and objectives found in the client's collaborative service plan. This service can be provided by a behavioral health professional, either individually or under clinical supervision.

  • Therapeutic Child Support (TCS)
    A mentoring program that works with children one-on-one in the community focusing on objectives set by the service team. TCS staff assist the recipient's family in understanding, treating, identifying or coping with the recipient's behavioral health disorder. This service can take on many roles and functions, such as monitoring a recipient's progress toward transition, assisting with the development of social skills, training in behavior management skills, providing in-home support, and mentoring to the recipient. This service can be provided by a person who has a bachelor's degree or a high school degree and who meets all the state qualifications and has a clean background check. This service is considered an "extension to individual therapy" and is supervised by a behavioral health professional, either individually or under clinical supervision.

  • Rehab Services
    Intrust Healthcare provides physical rehab services, including Physical, Occupational and Speech Therapy Services on a contractual basis. Rehab services are provided in a variety of settings, including Long-Term Care Facilities, Outpatient and Inpatient Facilities, Home Health and School settings. Intrust Healthcare's rehab services are provided throughout the Lake Cumberland and surrounding areas.
